Our Nurse Advocates are integrated into the fabric of an employer organization.  Different from other advocacy services, your dedicated Nurse Advocate is accessible, accountable to your organization and the experience to handle a wide range of clinical needs. With direct access, organizational familiarity and responsiveness, employees will have support needed to improve health outcomes be more productive.

Wellness Plan Administration

Our experience has shown that when a meaningful incentive is established, a significantly greater number of all eligible individuals will enroll. At the end of the first year more of these individuals will have followed the suggested intervention steps, improving their clinical risk conditions, and reducing the claims cost incurred to maintain their health.
Through either an onsite clinic or nurse advocate program, the employer has provided a significant resource to help employees. Synergy Healthcare provides the road map and incentives to encourage significant participation, health improvement and cost reductions.

On-Site Medical Care

Synergy Healthcare provides a personal and convenient way for plan members to identify their unique risks and proactively addresses them before they manifest themselves through expensive and often life altering acute care illnesses.  Our clinics range from 100 employees to 2,000 employees at a location.

Services include: Chronic disease management, writing prescriptions, diagnosis and treatment of minor medical conditions, laboratory services, medication dispensing, service referrals and health education counseling.

Physical Therapy

Managing musculoskeletal costs through an early intervention program can reduce one of your top claims while better supporting your employees.
Physical therapy is a highly utilized service in many of our clinics with over a 1,000 employees. We can set up a direct referral program with a high value community provider for clinics under 1,000 employees.

Autoimmune Health Coaching

3-5% of member population will be diagnosed with an autoimmune disease that is both ongoing and complex. Total autoimmune medical and Rx costs can be 15% of total plan spend.
Our functional medicine whole-body approach focuses on identifying and addressing the root cause of the health problems. While the program supports the “pillars of health”, which are diet, stress management, sleep/relaxation and movement, people with autoimmune issues have unique needs that can take extensive support and collaboration on solutions.
Only by addressing and correcting the causes of their disease will a patient gain lasting benefits and be far less likely to rely on an expensive biologic medication.
